Claims Procedure

  1. Download the following claim forms
  2. SUA Sports Injury Form

    i) have your club secretary sign the form

    ii) have your physician fillout and sign the "Attending Physicians Statement"

    The SUA form should be sent directly to SUA within 30 days of injury.
  3. Return the AFDA Injury Reporting Form and Medical Statement promptly to the AFDA, addressed to Member Compassionate Fund. The Injury Report Forms and Medical Statement should be sent within 30 days of the injury where possible. The Medical Expenses sheet can be emailed to admin@afda.com as receipts become available.
  4. You don't have to have all receipts, etc before lodging the claim. Lodge the claim and forward the receipts on after your treatment is complete.
  5. If you have private health insurance, you must claim from them first and then send us the rebate statement. Similarly, with Medicare (please retain a copy of any receipts to send to us).
  6. If you're not sure if you need to lodge a claim, do so anyway. Backing out later is far easier than lodging a claim too late.
